Summary

This funding opportunity is available to nonprofit organizations with tax‑exempt 501(c)(3) status (or equivalent), government entities or tribes, or organizations with fiscal sponsors . The grant is designed to support programs that improve community health outcomes, with particular emphasis on youth mental health (children, adolescents, and young adults) and access to direct healthcare services.


Awardees receive funding that supports one‑year contracts (for example, January 1 to December 31 of a given year) to carry out new or expanded services. Organizations must hold recognized tax‑exempt status (or have a fiscal sponsor) and serve the residents of the region in question.


Funding may be used for program delivery—such as clinical or non‑clinical mental health services, community‑based outreach and wellness initiatives, or projects that expand access to dental, medical or other direct care for underserved populations. The grant is not open to individuals or for profit businesses unless structured in partnership with a nonprofit; instead it is targeted at nonprofit agencies, community health providers and government entities.


The purposes of the grant are to reduce barriers to care and improve wellness by investing in organizations that have alignment with the stated priorities. Preference is given to programs that demonstrate measurable outcomes, strong community need, efficient implementation, and collaboration with other service providers.
